The Healthy Alliance SCN Resource Guide provides an overview of key operational policies, workflows, and contact information to support your participation in Healthy Alliance’s Social Care Network. It will serve as a centralized reference to provide clarity, promote consistency, and ensure alignment with SCN standards, while helping users navigate processes and expectations efficiently. You can feel free to navigate to any chapter within the guide as needed – no need to start at the beginning. This tool has been designed for ongoing use, therefore there is no need to mark as “complete.” We hope that you will return to it regularly to help inform your work.
