This training is designed for HRSN service providers and other Member-facing partners within the SCN ecosystem. Rather than focusing on systems navigation, this session centers on care management and engagement best practices. Participants will build practical skills to assess Member needs, develop and maintain meaningful care plans, coordinate services across providers, and support consistent follow-through over time—ultimately strengthening Member stability and sustainability.
